Stroke does not usually send a reliable list of warnings for weeks. The typical presentation is a sudden neurological change: a drooping face, loss of strength in one arm, words that will not come, vision that disappears or balance that abruptly fails. Other presentations include focal numbness, sudden trouble swallowing, acute confusion or an explosive, unusual headache. Stroke symptoms people commonly dismiss

Classic signs are easier to recognise when dramatic. They are missed more often when the person can still speak, there is no pain, or the episode ends before help is requested.

  • A curtain or loss of vision in one eye: this may be retinal ischaemia and carries vascular urgency even when sight returns.
  • Sudden double vision: especially with imbalance, abnormal speech, weakness or numbness.
  • Abrupt inability to stand or coordinate: this is different from familiar mild light-headedness.
  • A clumsy hand or focal sensory loss: suddenly dropping objects or clearly feeling one side differently deserves attention.
  • Language, not only pronunciation: nonsensical words, inability to understand a simple sentence or unexpected reading difficulty also count.
  • An episode that has already ended: a normal appearance afterwards does not rule out TIA or a small infarct visible on MRI.

Isolated dizziness, tingling, headache and brain fog are common and usually have other causes. This article is not meant to recast them as stroke. What raises urgency is abrupt onset, a localised deficit and other neurological changes.

TIA symptoms: five minutes does not make them benign

A TIA is a transient focal neurological episode attributed to ischaemia without acute infarction demonstrated on imaging. If MRI shows injury, it is classified as stroke even when symptoms have resolved. That distinction is made after assessment; at home the response is the same: emergency care.

A 2023 American Heart Association scientific statement notes that 90-day stroke risk after TIA can reach 17.8% in some cohorts, with almost half of events occurring within two days. A 2025 meta-analysis of 38 studies estimated a lower overall risk of about 5% at 90 days; in studies with more comprehensive neurological, brain, vascular and cardiac assessment it was around 3%. This does not prove that testing guarantees protection. It shows that risk varies and rapid assessment can identify treatable causes.

Urgent work-up commonly reconstructs the event, examines neurological function, measures glucose and blood pressure, images brain and vessels, and looks for a cardiac source such as atrial fibrillation. The exact test depends on the presentation. A home score or wearable cannot decide that it is safe to wait.

Months beforehand, the issue is usually silent risk—not symptoms

The question “does a stroke warn you?” combines two situations. A TIA is an immediate clinical warning. Hypertension, diabetes, kidney disease, sleep apnoea, atrial fibrillation, smoking and atherosclerosis can instead be present for years without a recognisable signal.

Atherosclerosis is plaque accumulating in arteries and may affect coronary, carotid, cerebral and peripheral circulation. A coronary calcium score describes coronary plaque; it neither diagnoses a cerebral blockage nor says a stroke is imminent. ApoB and lipoprotein(a) can refine atherogenic risk, but they are not stroke clocks.

If your thyroid or hormones are already abnormal, does prevention change?

It may, but the diagnosis matters. “Hormonal imbalance” is not one condition: overt hyperthyroidism, mildly raised TSH with normal T4, early menopause, oestrogen therapy and diabetes do not carry the same implications. Precision begins by naming the condition, not expanding the panel.

A meta-analysis of 11 cohorts found that overt hyperthyroidism was associated with more stroke than euthyroidism (HR 1.35; 95% CI 1.03-1.75). This was observational evidence with moderate heterogeneity; it does not prove that treating thyroid disease reduces stroke by the same proportion. It does support avoiding endogenous hormone excess and overtreatment, checking rhythm when palpitations or tachycardia occur, and managing documented atrial fibrillation according to risk.

Mild results tell a different story. An individual-participant analysis of 47,573 adults found no clear overall increase in stroke with subclinical hypothyroidism (HR 1.05; 95% CI 0.91-1.21), although uncertain signals appeared in younger subgroups. Among 100,250 adults, positive TPO antibodies were not associated with stroke either (HR 0.98; 95% CI 0.87-1.11). Thyroid antibodies should not be ordered to calculate cerebrovascular risk.

Overt hypothyroidism can worsen cholesterol, ApoB, blood pressure or weight in some people. Treating it has its own indication and may improve part of that profile; it does not replace vascular treatment when risk remains. If you take thyroid hormone, the practical goal is to avoid under- and overtreatment through symptoms, TSH/free T4 when appropriate, stable dosing, interaction review and planned follow-up.

For women's hormonal health, the 2024 AHA/ASA guideline includes premature ovarian insufficiency before 40, early menopause before 45, endometriosis and adverse pregnancy outcomes as reasons to revisit vascular risk. That does not make menopausal hormone therapy a stroke-prevention treatment. Decisions about menopausal hormone therapy depend on symptoms, age, time since menopause, route, dose, migraine, smoking, blood pressure, thrombosis and history; do not start or stop it after reading a checklist.

Habits, follow-up and precision: what can help

Precision medicine is not ordering every test. It means finding the risk mechanism that could change a decision, applying a proportionate intervention and checking whether it worked. Blood pressure will often matter more than an exotic biomarker.

  1. Define the scenario. Primary prevention, documented atherosclerosis, atrial fibrillation, previous TIA and previous stroke are not interchangeable; each changes targets and medication.
  2. Confirm repeatable data. Properly measured blood pressure, lipids with ApoB when useful, glucose/HbA1c, kidney function, smoking, activity, medicines and family history. ECG or rhythm monitoring is used when history or risk supports it.
  3. Integrate endocrine health without shortcuts. With thyroid disease, diabetes, early menopause or another hormonal condition, review control, treatment and the effects on pressure, rhythm, lipids or thrombosis.
  4. Choose an intervention with a dose. WHO advises at least 150 minutes per week of moderate activity, adapted to function and safety. A genuine Mediterranean pattern prioritises vegetables, fruit, pulses, nuts, olive oil and fish while reducing salt, tobacco and harmful alcohol use.
  5. Set a review date. An intervention without follow-up is not precision. Recheck adherence, tolerance, pressure, lipids, glucose, rhythm or thyroid function at the agreed interval and adjust.

In PREDIMED, 7,447 Spanish adults aged 55-80 with high cardiovascular risk but no cardiovascular disease at baseline received quarterly sessions and a Mediterranean diet supplemented with extra-virgin olive oil or nuts. Over a median 4.8 years, the composite of myocardial infarction, stroke or cardiovascular death was lower than with low-fat advice: HR 0.69 with olive oil and 0.72 with nuts. This was a sustained dietary pattern in a high-risk population, not the effect of adding one food for a few weeks.

Blood-pressure reduction improves clinical outcomes rather than merely producing a better number. An individual-participant meta-analysis of trials found that each 5 mmHg reduction in systolic pressure with treatment reduced major cardiovascular events by roughly 10% and stroke by about 13%, with absolute benefit depending on baseline risk. Targets and drugs require individualisation because excessive lowering may also cause dizziness, falls or kidney injury in some people.

Sleep deserves attention because sleep apnoea, nocturnal blood pressure and metabolic health connect to vascular risk. But poor sleep, stress and fatigue do not announce a stroke. What preventive care cannot do

  • It cannot guarantee that stroke will not occur.
  • It does not make whole-body MRI, carotid ultrasound or a hormone panel universal screening.
  • It does not justify treating a plaque, thyroid result or number without weighing benefit and harm.
  • It cannot replace emergency care when symptoms occur, even after a recent normal check-up.
  • It does not justify self-starting aspirin, anticoagulation or supplements.

Preventive medicine can recognise risk before symptoms; it cannot predict the future. A useful personalised health plan records which risk is being treated, who owns follow-up, what will be measured, when it will be reviewed and which finding should change the plan.

Red flags: one simple rule

Any sudden loss of neurological function is an emergency until proved otherwise. Call emergency services for facial droop, one-sided weakness or numbness, altered speech or understanding, sudden vision loss, disabling imbalance, incoordination, new trouble swallowing or an explosive headache. Note the last-known-well time. Do not wait for pain, recurrence or the next morning, and do not drive.
